The skull basement is very clearly the final dungeon. You just need the required items to be able to enter, which would probably be some sort of rolling bed or wheelchair to reduce weight loss.

Now we've just got to figure out what opponent he has to beat down there. With his luck, probably a dinosaur.

Especially in that second to last panel, he looks like a raptor bug and I love it.

Yeah, that's an awesome design! Markas has gone from a background character that nobody knew to a standout villain in the series over only a couple pages! I like seeing background characters get the spotlight, especially when they end up looking cool like this.
